Floral Home Care agency is looking to hire experienced Physical Therapists (PT) and Occupational Therapists (OT)ResponsibilitiesIdentify and meet patients needs and goals.Develops a plan of care for the patient identifying short and long term goals and patient and caregiver involvement.Observes, records and reports to the physician and the nurse, the patient reaction to treatment and any changes in the patient condition.Familiar with stretching, soft tissue mobilizations, myofascial release, manual therapy and corrective exercisesConsult and practice with other health professionalsMaintains communication with physician and other personnel involved in the patient care, through written reports, attendance at meetings and confirmed telephone calls.Completes initial and ongoing assessments to determine patient level of functioningRequirements:* NYS PT/ OT LicenseWe offer Great Pay with room for growth.Please send your PT/OT resume and we will contact you.
